This information is being collected by HENRY, a registered charity (Charity No. 1132581) in partnership with Best Start in City and Hackney. HENRY develops and supports services to help families get a healthy start.

The data will be anonymised and shared with the provider organisation (London Borough of Hackney & City of London Corporation Public Health Teams), for the purposes of reporting on service effectiveness, evaluation of the achievement of population-level objectives, and continuous improvement.

We will use your contact details to get in touch so we can discuss what would be best for you and arrange support. This information will be retained for as long as necessary to get in contact with you relating to the HENRY service and for 7 years after that, after which it will be permanently deleted.

Under the Data Protection Act (2018) you have the right to access, rectify or erase the data we hold on you. You also have the right to object to or restrict the way we use your personal information.
